The Best Mattress Toppers for Back Pain in 2026

I slept on eleven of them over fourteen months, on the same sagging queen mattress that started all of this. Only one of them was still on my bed at the end.

Here is the part nobody tells you about a mattress that has started to sag: it does not hurt when you buy it, and it does not hurt on the night it gives up. I ordered this one late, and reluctantly. By month nine I had convinced myself that fibre toppers were the cheap option โ€” the thing you buy at a big-box store when you cannot afford foam. I was wrong about that, and it cost me about six months of bad sleep.

The first thing you notice is what it does not do. It does not swallow you. Let me be fair to TEMPUR-Pedic, because this is genuinely the best piece of foam in the test and it is not close. The density is in a different league from the $50 slabs; you can feel it in your hand before you ever put it on a bed. On pure pressure relief it scored higher than my winner. If your problem is a hip or a shoulder that goes numb on your side, this is the most effective thing here.

Two things stopped it. The first is heat. Three inches of dense memory foam under your whole body is three inches of insulation, and by night four I had worked out that I was waking up not because of my back but because I was too warm. TEMPUR sells a cooling version for another fifty dollars, which tells you they know.

The second is the trench. Dense foam contours beautifully and then keeps the shape for a beat too long. If you are someone who shifts a lot at night โ€” and people with back pain shift a lot at night โ€” you spend the small hours climbing out of your own outline.

At $221.80 for the queen it is also the most expensive thing on this list by a factor of four over the cheap end.

The egg crate is the oldest back-pain product in the category and it is still everywhere, which is usually a sign that something about it works. It does. The peaks compress under the heaviest parts of you and the valleys let air move, so it runs cooler than a solid slab, and the "7 zones" โ€” firmer under the lumbar, softer under the shoulder โ€” are not pure marketing. For fifty dollars it is a lot of relief.

The problem is what happens to convoluted foam over time. Those peaks are thin columns of foam carrying your body weight every night, and they do not come back. Mine had visibly flattened under the hips at around week ten. By the time I retired it, the zoning I had paid for was a memory.

It also cannot be cleaned. Convoluted foam is impossible to wash and slow to dry, and mine came with a cover you can remove but a core you cannot do anything about.

The featherbed is the most seductive product in this test and the worst one for the specific problem this article is about. Night one is genuinely lovely. It is the hotel feeling, the one you get in a good room and spend the next year trying to reproduce at home.

By night four you find the flaw. Feathers have no structure โ€” they compress and stay compressed, and they migrate away from the places you press hardest, which are exactly the hip and the shoulder. So the dip in your mattress does not get corrected. It gets upholstered. You are lying in the same bad shape, on something softer.

Mine also needed shaking out every single morning, and no matter how good the shell is, feathers work their way out and end up in the sheets.

This is the topper most people buy first, and I understand why: it is cheap, it is on every marketplace, it has tens of thousands of reviews, and the words on the box are the right words โ€” gel infused, cooling, pressure relieving, CertiPUR certified.

It is fine. That is the honest verdict. For sixty-odd dollars you get a real layer of foam between you and a tired mattress, and for the first fortnight it is a clear improvement on nothing.

What you also get is every memory foam problem in a lower-density package. It ran hotter than the TEMPUR, not cooler, because gel infusion changes the first twenty minutes and not the night. It compressed under the hip faster because the density is lower. And it cannot be washed.

There is also the thing you notice in the box: it arrives vacuum-rolled and takes 48 hours to expand, and the version that comes out is thinner than the version on the packaging. That gap between the advertised inches and the delivered inches is the single most common complaint across this entire category, at every price point, and it is worth knowing before you order anything.
